Request for Appropriation of Funds Form

The following is a list of items that along with the enclosed form should be submitted to the Parks and Recreation Department before your yearly allotment is issued:

1. Federal Tax I.D. Number
2. Current By-laws and Constitution
3. Current Officers and Board of Directors (including telephone numbers & addresses)
4. Schedule of league meetings (including dates and times)
5. League and practice schedule (including list of all practice sites)
6. Copy of insurance form
7. Emergency Medical Procedures in case of injury during the playing season.

Please include all of the above and return to the Parks Department as soon as possible.
