Appropriation of Funds - Request Form: The following items MUST be submitted along with the request form to the Parks & Recreation Dept. before your yearly allotment is issued:
- Federal Tax I.D. Number
- Current By-laws and Constitution
- Current Officers and Board of Directors (including telephone numbers & addresses)
- Schedule of league meetings (including dates & times)
- League and practice schedule (including list of all practice sites)
- Copy of insurance form
- Emergency Medical Procedures in case of injury during the playing season
