Asst. Clerk | Passport applications may be obtained at the Southington Probate Court. The fee for a new or renewed passport is $75.00 for an adult and $60.00 for a minor. The fee must be paid by check or money order. In addition, there is a $25 probate fee for processing the application, which may be paid by cash or check. Passports for children under 14 years of age must be accompanied by a long-form birth certificate and both parents must sign the application. Passport renewal forms are available at the Southington Probate Court. A passport may be renewed by mail under the following conditions: - Applicant can submit his or her most recent passport.
- Applicant was 16 years or older when the most recent passport was issued.
- Most recent passport was issued less than 15 years ago.
- Applicant uses the same name or can submit proper documentation to reflect name change.
